PROCEDURE FOR REPORTING UNSAFE PRACTICE OF ODP STUDENTS
Whilst working with Operating Department Practice Students in the clinical environment, you may observe them performing practice that you deem to be unsafe. This type of incident must be recorded and dealt with accordingly and as such Supervising Mentors must report such incidents to UCS.
The procedure for reporting unsafe practice is as follows.
PROCEDURE FOR REPORTING UNSAFE PRACTICE OF ODP STUDENTS
UCS must be informed of all incidents in which a student ODP has demonstrated unsafe practice.
The following process must be followed:

The incident must be dealt with immediately to protect all parties involved

The supervising mentor witnessing the unsafe practice must complete the Unsafe Practice Report Form provided by UCS as soon as possible after the incident. Click this link for a copy of the ODP Unsafe Practice report form.

Once the form has been completed it must be discussed with the student and the clinical supervisor

The clinical supervisor must ensure that the supervising mentor, the student and the theatre manager has signed the form

Appropriate feedback and support must be given to the student following the incident

Each incidence of unsafe practice will be logged and action will be taken according to the nature of the unsafe practice demonstrated

Any incidence of an unsafe practice may result in the student being subject to disciplinary action

A student who is reported to have demonstrated unsafe practice on three separate occasions may result in disciplinary action being taken

On completion, this form must be forwarded to Claire Webb, Head of Allied Professions and Science accompanied by any other evidence if necessary
