How will I be assessed?
The assessment for this module consists of two components: -

2000 word client-centred case study focused to an individual with long term condition(s)
weighting 50%

2000 word written report that critically promotes the on going management of long-term conditions on behalf of a client or client’s family.

If you are already registered on a diploma or degree programme it is advisable to inform your course leader that you will be undertaking this module. This will enable your course leader to undertake the vital task of tracking your academic journey.
