Management of the Individual with Long Term Conditions

Overview

This module aims to:

  • Explore and critically analyse the provision of interventions to address selected long-term conditions.
  • Apply a proactive, individualised, whole-person approach to the care of people with long - term conditions, using primary care as the central organising force and with all interventions focused on promoting maximal function, independence, comfort, and quality of life.
  • Explore roles and responsibilities within the case management of clients with long term conditions.
  • Enhance the case management of long term conditions

Exploring work – based learning needs and procuring stakeholder support
Report writing and presentations
The chronic disease model
Definitions of case management
Interagency and partnership working
Leading complex care coordination
Proactively managing complex long term conditions
Supporting self - care, self - management and enabling independence
Decision - making and challenging barriers
Identifying high risk people, promoting health and preventing ill health
Managing care at the end of life

How will I be assessed?

The assessment for this module consists of two components: -

  • 2000 word client-centred case study focused to an individual with long term condition(s)
    weighting 50%
  • 2000 word written report that critically promotes the on going management of long-term conditions on behalf of a client or client’s family.
    weighting 50%

If you are already registered on a diploma or degree programme it is advisable to inform your course leader that you will be undertaking this module. This will enable your course leader to undertake the vital task of tracking your academic journey.
