Case Management Competences Portfolio

Overview

This module is designed to support those practitioners from the allied health, social care or nursing professions who are also undertaking the module Case Management of Long Term Conditions (Honours Level) and who wish to achieve the Case Manager Credential.

 

Content and Modules

Level 6 (Undergraduate)

Pre-requisites: Access to relevant clinical experience with patients/clients. Diploma in health or social care profession / 120 credits at intermediate level. Procurement of clinical mentor.

Aims

This module aims to enable practitioners to:

  • Demonstrate effective case management of long term conditions within the domains laid out in the Case Management Competencies Framework (NHS Modernisation Agency 2005). These learning outcomes are derived from the Case Management Competencies Framework for the Care of People With Long Term Conditions (NHS Modernisation Agency 2005).

Learning Outcomes

By the end of the module the practitioner will be able to demonstrate competency in the following domains of case management:

  • Leading Complex Care Coordination
  • Proactively Manage Complex Long Term Conditions.
  • Managing Cognitive Impairment and Mental Well Being
  • Supporting Self Care, Self Management and Enabling Independence
  • Professional Practice and Leadership
  • Identifying High Risk People, Promoting Health and Preventing Ill Health
  • Managing Care at the End of Life
  • Interagency and Partnership Working

Summative Assessment

A portfolio demonstrating that knowledge and understanding has been applied by the practitioner according to each of the domains in the case management framework and all competence have been achieved.
